DISABILITY / COMMUNITY SUPPORT WORKER 

We provide variety of services to our clients as: 

  • Long Term Accommodation- Independent Living built around our participants’ needs to enable them to live more independently. 
  • Respite service- allows people living with disability to experience time away from home in a safe and supportive environment 
  • Community Participation- is conducted by qualified and experienced staff under the direction of HHCC management We work with each client, their family and carers to identify personal goals for engagement in the program.  
  • In -Home and Personal Care Services- supporting our clients who live in their own homes and require assistance with daily living tasks 
  • Day Program- you will have a unique opportunity to try new activities in a caring environment where each individual is given all the support they need. 
  • Allied Health Services- Allied Health Specialists who work with you to give you the best care possible so that you can live a healthy life. 

KEY OBJECTIVE OF THE SERVICE/DEPARTMENT: 

Provide people with disability with opportunities for continued learning and life skill development and participation in the community, which increase their independence and ability to meet their life goals.

ESSENTIAL QUALIFICATIONS & EXPERIENCE REQUIRED
 

 First Aid & CPR Certificate 

  • Certificate III in Individual Support or relevant qualification in the industry 
  • Working with Children’s Check 
  • National Police Check 
  • Australian Driver’s Licence 
  • Knowledge of the NSW Disability Standards
  • Knowledge of WHS principle including safe manual handling procedures
  • Ability to drive 12-seater van desired

    DUTIES/RESPONSIBILITIES 

     Establish effective communication with customers using whatever means of communication is appropriate for each individual. 

    1. Consult with customers and take account of their wishes in all matters relating to their care with the proviso that Duty of Care and WHS issues must also be considered. 
    2. Implement and review of Individual Plans Program. 
    3.  Record the progress of each customer in line with relevant documentation. 
    4. Provide transport for customers or assistance to use public transport. 
    5. Provide physical assistance as required.
    6. This may include: Assistance with meals. 
      • Assistance with toileting. 
      • Transfer/hoisting between toilet, wheelchair, motor vehicle etc. 
      • Medical/special procedures where required. 
      • Administering medication where required. 
      • Physical assistance to participate in activities 
      • Housekeeping duties. 
      • Devise and facilitate program activities and workshops in conjunction with customers and other support staff. 
      • Support and actively encourage customer’s participation in decision-making about their own program and about the service. 
      • Complete documentation as required. This may include: 
      • Customer notes 
      • Shift reports 
      • Learning logs/Monthly reports 
      • Risk management/Positive behaviour support 
      • Medication forms 
      • Timesheets 
      • Attend staff meetings, supervision meetings, training and other relevant meetings as required 
      • Perform the necessary duties to ensure that the premises and vehicles are kept clean and tidy. 
      • Contribute to individual service planning and review. 
      • Maintain a professional standard of behaviour in keeping with the HHCC Code of Conduct. 
      • Work effectively as a part of a team and utilise grievance procedures if problems arise. 
      • Utilise a Positive Behaviour Support approach when working with customers, to follow HHCC Positive Behaviour Support policy and procedures, and to participate in the development, implementation and record keeping for all related plans and applicable to their role. 
      • Comply with, report on and actively participate in HHCC WHS.
